We offer several plans based on your individual needs:

    PLAN A:

  • $300/year (a $123 savings + 25% off additional treatments!)
  • $250/year – patients 12 and under

    • Includes:

    • Two exams/year with Dr. Eghtessad
    • Two regular cleanings
    • One set of Bitewing cavity-detecting x-rays
    • Two fluoride treatments
    • 25% off of ANY additional treatments

    PLAN B:

  • $560/year (a $225 savings + 25% off additional treatments!)

    • Includes:

    • Two exams/year with Dr. Eghtessad
    • Four periodontal maintenance cleanings
    • One set of Bitewings cavity-detecting x-rays
    • Two fluoride treatments
    • 25% off of ANY additional treatment

  • Payment must be paid in full prior to first dental cleaning appointment.

  • All cleanings must be used within one year of purchasing the plan or offer becomes void and other cleanings are forfeited.

  • Failure to keep your scheduled appointments will result in cancellation fees. This offer cannot be transferred or combined with other promotional offers or discounts.

  • Appointment cancellations must be made at least 48 hours in advance to avoid $75 cancellation fee.
